If some player hate you or you need something new you can teach me

I have a few questions on the Application Form for a higher rank, and these are the questions i would like to ask.

Here is the Application Form:

How can you do ? :
Why you apply for higger rank :
What server you want higger rank ?
If you find bad players what you will do ?
Do you like me ?
Where you life ?

Question 1: In the Name area, do you have to write your real name or your in-game-name (ign)?
Question 2: In the How can you do area, what does that mean?
Question 3: In the Where you life area, again... what does that mean?
* does that ask where you live?
* or does it ask where you were given birth on?
* or does it ask where you were given life?
Question 4: can I edit the Application form?

Answer 1: in Name area Maybe its your in game name (ign).
Answer 2: How Can You Do its what its you can do if you a Mod/Admin what you can do in server.
Answer 3: Where You Live? its mean where your country/where your from.
Answer 4: i dont known.
